Contamination Control in Hospitals and Care Homes

Hospitals and nursing homes are among the most contamination-sensitive environments, where infection prevention plays a crucial role in ensuring patient safety and maintaining high standards of healthcare quality.

Every day, healthcare professionals, patients, visitors, medical devices, and mobile equipment continuously move throughout these facilities, increasing the risk of contaminant dissemination.

Unlike many other controlled environments, contamination in healthcare settings has direct implications for patient health. Pathogens can spread through contact with contaminated surfaces, footwear, medical devices, and environmental reservoirs, contributing to the transmission of healthcare-associated infections (HAIs), also known as nosocomial infections.

The role of Floors in Contamination Control

Hospital and nursing home floors have traditionally been considered low-risk surfaces because they are rarely touched directly by patients or healthcare workers. However, recent studies suggest that floors may represent an underestimated reservoir for pathogen dissemination within healthcare environments.

A study published in the American Journal of Infection Control found that patient room floors in five hospitals were frequently contaminated with healthcare-associated pathogens, including multidrug-resistant organisms. High-touch items such as blood pressure cuffs, medical devices, personal belongings, call buttons, and bed linens were often found in direct contact with the floor. Researchers also demonstrated that handling objects that had been in contact with contaminated floors frequently resulted in pathogen transfer to hands.

More recent studies have shown that contaminated floors may act as the initial site of pathogen dissemination. Pathogens recovered from floors have subsequently been detected on patients' socks, bedding, and high-touch surfaces, highlighting how contamination can progressively spread throughout patient care environments.

Medical devices are not exempt from this contamination pathway. Equipment used in routine patient care is frequently moved between rooms and may come into contact with contaminated flooring, increasing the risk of indirect transmission if appropriate contamination control measures are not implemented.

Every movement within healthcare facilities contributes to the introduction and redistribution of contaminants. Shoes, wheelchairs, stretchers, medication carts, and mobile medical equipment continuously transport particulate matter and microorganisms between departments and patient care areas. Once introduced into critical environments, contaminants can easily spread to surrounding surfaces and personnel.
